Job Summary: 

The Memory Care Coordinator is responsible for the day-to-day management and success of Reflections Memory Care. Must be knowledgeable and current on information concerning Alzheimer's disease and other types of dementia.

Requirements: 

  • Must have a minimum of 3 years Supervisory experience and 3 years of Recreation Activity experience in assisted living, long term care or similar industry setting.
  • A Certified Dementia Practitioner (CDP) and Certified Therapeutic Recreation Specialist (CTRS) with a bachelor's degree is preferred.
  • Must be experienced and knowledgeable about the dementia population.
  • Must have excellent verbal and written communication skills.

Duties: 

  • Plans, organizes, develops, implements, and interprets the programs, goals, objectives, policies, and procedures.
  • Responsible for staffing, scheduling, and coverage for Reflections Memory Care.
  • Arranges monthly support group meetings and educational opportunities for families.
  • Maintains regular communication with families through use of phone calls, emails, text messages, care plan meetings, etc.
  • Collaborates with Activity Assistants to monitor resident engagement.
  • Maintains care plans upon move-in and updates every six months.
  • Designs, schedules, and facilitates the Reflections Program that incorporates life skills, activities and creating happy days.
  • Use of Daily Reports for Reflections and 24-Hour Reports.
  • Follows emergency operations plans and procedures in the usage of firefighting equipment and ensures resident evacuation of the building.
  • Maintains current CPR certification and First Aid certification.
  • Is an active participant during state survey ensuring regulatory compliance of practices are met by Reflections staff.
  • Prepares bi-weekly payroll hours and submits to Payroll in a timely manner.
