Job Summary:  The Patient Navigator functions as a clinical support liaison between patients, providers and care team members throughout Osborn Family Health Center and reports to the Director of Nursing.  Also coordinates patient-centered care as a liaison between referring physicians, hospital facilities and all other collaborating health care entities.  This position will also work in collaboration with any and all applicable departments. 

 

Summary of Accountabilities:

  1. Always provides high quality customer service to patients in person and over the telephone.
  2. Maintain current demographic information of patients (name change; address; insurance updates).
  3. Always maintains strict HIPAA compliance.
  4. Fluent in Spanish/English.
  5. Ability to assist with patients who present with language barrier- Translation Video/Translation of Telephone.
  6. Ability to be detailed oriented and organized.
  7. Assist in the navigating of patients to reduce gaps in care.
  8. Track and monitor provider orders for outpatient services to include laboratory, radiology, diagnostic testing, and consultation services.
  9. Attach results and/or consult reports for outpatient services including laboratory, radiology, diagnostic testing, and consultation services for provider review.
  10. Work to improve patient compliance through telephone calls, written correspondence, and the patient portal.
  11. Assist patients, when necessary, the copies of lost or misplaced orders upon request.
  12. Notify ordering provider of patient non-compliance.
  13. Facilitate the process for follow-up appointments as needed. 
  14. Willingness to engage in training, i.e., webinar, webcast, any on-line opportunities.
  15. Ability to communicate to promote effective working relationships with other members of staff.
  16. Make recommendations on how to improve efficiency in departments.
  17. Ability to work effectively under pressure.
  18. Willingness to cooperate with covering of staff members and/or making recommendations to manager.
  19. Ensure compliance with all company policies, procedures, and guidelines.
  20. May sometimes undertake a variety of special projects and/or administrative duties to assist in the efficiencies of the organization.
  21. Communicate concerns/patient issues with Director of Nursing and/or Risk/Claims Manager.
  22. Assists with any additional duties as needed or as directed by Director of Nursing.

Minimum Certifications, Registry and/or Licenses Required:  NA

 

Minimum Degree Required and Experience Required:

At least one year as a Certified Medical Assistant or any combination of education and experience that would likely provide the required knowledge, skills, and abilities as well as possession of any required certifications.

 

Skill Requirements:

Skill in establishing and maintaining effective working relationships with other employees, patients, hospital departments, organizations, and the public. Strong inter-personal and communication skills. Good command of the English language both orally and written.  Ability to perform diversified duties with a high degree of accuracy. 

Knowledge of Referral/Authorization Processing for Insurance Company preferred. 

 

 

Physical Requirements:

Ability to see and hear within normal limits with or without use of corrective devices

Motor dexterity of hands and fingers.

Ability to utilize proper body mechanics

Ability to lift at least 50 pounds (without assistance)

Ability to frequently bend, lift, move and pull. Ability to stand and walk for long periods of time.

Environmental Conditions:

Interior office setting, examination rooms, etc.  The worker is subject to inside environmental conditions
