Child and Adolescent Intensive In-Home Care Coordinator

This is a Bachelors' level position that works as part of a two-person team to provide direct in-home intervention for severely emotionally/behaviorally disturbed children and their families. This Child and Adolescent Team will have a maximum caseload of six consumers, for up to twelve weeks. The primary mission of the intervention is to reduce a crisis, institute a therapeutic process, and prevent out-of-home placement of the child. Children and adolescents are defined as anyone aged five to seventeen. Services are provided primarily in the consumer's home and community. The team will primarily cover a specific county or counties within the catchment area; however, teams may be assigned to cases in any of the 4-county region (Montgomery, Lowndes, Autauga, and Elmore). Specific assignments will be discussed if a job offer is made.

 

DESCRIPTION OF DUTIES:

Provide the following services as part of a two-person team to include medication monitoring, mental health consultation/coordination, psychoeducational services, teaching Basic Living Skills, and 24/7 Crisis Intervention availability.

Serve in a supportive role in individual and family therapy sessions as lead by master's level therapist.

Responsible for assessing needs (CANS) and participating in the development of service plans for children and adolescents with SED (Serious Emotional Disturbance) and their families.

Ensure CANS assessments and Crisis Stabilization and Support Plans are updated in a timely manner according to ADMH standards.

Coordinate services treatment providers to include but not limited to the Mental Health Therapist and Care Coordinator, Certified Peer Specialists and/or Therapeutic Mentors as part of the referral process and throughout the treatment period. 

Responsible for documenting all services and pertinent information in the electronic health record.

Ensure all reporting requirements are met in a timely manner. 

Team members must meet a combined productivity requirement of 80% of their time as billable services. 

Refer the individual for such other services as may be appropriate.

Must work closely with Department of Human Resources, Family Court and other agencies in treatment provided to children and their families during the in-home intervention.

Provide transportation to children and families when appropriate.

Perform other duties that are assigned by the supervisor.

Participate in weekly and as-needed clinical supervision. 

 

REQUIREMENTS:

Knowledge of the special needs and behavioral characteristics of severely emotionally/behaviorally disturbed children and their families.

Knowledge of psychotropic medications to include their use and their side effects.

Considerable knowledge of mental health services and available resources.

Ability to interact and communicate effectively with other professionals and other agencies.

Ability to organize and manage one's own activities with a minimum of direct supervision.

Ability to work flexible hours due to the special needs of children and their families.

Ability to quickly establish rapport with clients.

Ability to establish and maintain good working relationships with consumers, families, hospital staff and community mental health center staff.

Ability to work in a proactive, assertive, and energetic manner in fulfilling the duties of this position.

Ability to work effectively as a treatment team member.

Reliable transportation and willingness to transport clients.

Ability to work flexible schedule including evenings and weekends if necessary.

Willing to work in non-traditional and in-home settings.

 

QUALIFICATIONS:

Bachelor's degree in behavioral science or related field from a recognized college or university. Some experience preferred.

Must meet requirements for Approved Medicaid Provider status.

Must complete and receive certification in DMH approved in-home and case management training upon hire.

If hired, must obtain Red Cross First Aid, CPR, and CPI certifications within first week of orientation. Must maintain current certifications at all times.

Must hold a valid driver's license and maintain a driving record that is acceptable to Carastar's insurance carrier.

Must maintain at least liability coverage on personal vehicles.
