Provides a broad range of paraprofessional social services to students in the
GAP Program and their families. Conducts Intakes, participates In treatment
team planning (updating treatment plans as needed and working toward goals).
Will work with teachers, family members,& guidance counselors to provide the
best quality services possible to students on caseload. Assists in dealing with
personal & social problems. May need to provide supportive counseling

Position will be completing Authorization requests, CANS as needed, insurance
checks as needed, completing orders for services as needed, completing well
checks as needed, scheduling appointments for BHA/BHR as needed, and other
duties that would be assigned.

Minimum Qualifications:
Bachelor's degree in rehabilitation, social work, education or a related area
from an accredited college or university.
