GENERAL NATURE OF WORK: Under general supervision, assesses, screens and
counsels clients in a variety of program areas performing social services
activities to enhance, meet or restore their functioning capacity. Develops or
participates in, case management/care plans. Provides case management/case
coordination services to consumers. May supervise Para-professional level staff.
70% Productivity is required.
