Adult In-Home Therapist

This position involves providing direct in-home intervention for individuals with Serious Mental Illness or who are in immediate crisis. The employee will be a member of a two- or three-person team that will provide time limited, home-based services. The primary mission of home-based intervention is to reduce a crisis, engage clients in intervention services, and prevent out of home placement of the consumers. Services are provided primarily as a team.

 

DESCRIPTION OF DUTIES:

Assess consumers and individuals in crisis and implement clinical and care coordination services.

Coordinate with Carastar team members and community resources to link individuals to the appropriate level of care.

Provide direct in-home intervention services to include individual and family therapy and case management.

Perform the function of a mental health therapist by providing individual mental health services to each client on the team’s caseload.  Caseloads will not exceed 20 in-home consumers.  Services include evaluation/assessment, individual and group therapy/counseling, medication monitoring and education, family services, and other clinical services, focusing on mental disorders, environmental stress, and family relationships.

Team must meet a combined team productivity requirement of 80% direct services.

Provide in-home services to consumers.

Participate in staffing of all assigned cases on a regular basis with other treatment team members.

Participate in clinical supervision as required.

Participate in training activities as required by Carastar to increase and enhance professional and therapeutic skills.

Conduct initial intake interviews of clients according to established procedure, assess the client's problems and needs, and develop treatment plans using strength based, client-centered interventions.

Update treatment plans as appropriate and review objectives, goals, and interventions.

Ensure consumers are transitioned to a lesser or greater restrictive level of care when appropriate.

Provide mental health consultation to other agencies and providers assisting Carastar's consumers.

Participate in discharge planning within the crisis units as part of a warm handoff to in-home team services.

Document all services within the Electronic Health Record.

 

REQUIREMENTS:

Considerable knowledge of individual and group behavior with special emphasis on mental and emotional disorders.

Knowledge of principles and methods of counseling.

Knowledge of psychotropic drugs concerning their use, misuse, desirable/undesirable effects, and research materials available on these drugs.

Knowledge of assessing the mental status of individuals in need of mental health services.

Considerable knowledge of mental health services and available resources.

Ability to establish and maintain good working relationships with consumers, families, hospital staff, and community mental health center employees.

Ability to work in a proactive, assertive, and energetic manner while fulfilling the duties of this position.

Ability to work effectively as a treatment team member.

Must complete and receive certification in DMH approved in-home and case management training upon hire.

Reliable transportation and willingness to transport clients.

Ability to interact and communicate effectively with other professionals and other agencies.

Ability to organize and manage one's own activities with a minimum of direct supervision.

Ability to work a flexible schedule, including evenings and weekends if necessary.

Willing to participate in on-call services.

Willing to work in non-traditional and in-home settings.

Ability to quickly establish rapport with clients.

 

QUALIFICATIONS:

Master's Degree in behavioral science from a recognized college or university with a clinical practicum.

ALC, LPC, LGSW, or LCSW preferred (see note below)

Some clinical experience preferred.

Degree and experience must meet requirements for Approved Medicaid Provider status.

Must hold a valid driver's license and maintain a driving record that is acceptable to Carastar's auto insurance carrier.

Must maintain at least liability coverage on personal vehicles.

Must hold and maintain a current Red Cross First Aid, CPI and CPR certificate.

Therapist Level Hourly pay rate scale:

Therapist I- Unlicensed Therapist- $16.78 - $25.53/hour

Therapist II- Provisionally Licensed (ALC, LMSW, etc.)- $17.20 - $26.17/hour

Therapist III- Fully Licensed (LPC, LCSW, etc.)- $19.23 - $29.26/hour
