QUALIFICATIONS:

 

Knowledge of nursing policies, procedures, protocols, treatments, and standards of care. Knowledge of operations of regulatory agencies.  Documentation skills meeting legal and professional standards of care. Knowledge of HIPPA and confidentiality laws.  Ability to assess, plan, and provide basic nursing skills.  Knowledge of procedures including developing, implementing and revising plan of care. Ability to plan for unit needs.  Ability to determine if patient has received appropriate care.  Knowledge of federal and state guidelines governing programs.  Knowledge of community programs.  Skill to serve as a preceptor to peers.  Ability to do case management.

G.  ASSIGNED DUTIES

 

Plans, implements, and evaluates nursing care under the supervision of RN. Assess client health problems and needs, develop and implement nursing care plans, and maintain medical records. Oversees the provision and quality of nursing care to ill, injured, convalescent, or disabled clients.  Advises clients on health maintenance and disease prevention and provide case management.  Administer ordered injection and work closely with psychiatrist.

 

Duties Include:

 

70%   Consults and coordinates with health care team members to assess, plan, implement, and evaluate client care.  Monitors all aspects of client care, including diet and physical activity. Observes nurses and visits clients to ensure that proper nursing care is provided.  Prepare clients for labs, injections and other orders made by MD, and assists with examinations and treatments.  Maintain productivity between 50 to 70%

 

20%   Maintains accurate, detailed reports and client records.  Modifies client treatment plans as indicated by clients' responses and conditions.  Monitors, records, and reports symptoms and changes clients' conditions. 

 

5%     Prepares visits, sterile instruments, equipment and supplies, and ensures that stock and supplies is maintained.  Provides health care, first aid, immunization and assistance in convalescence and rehabilitation.

 

5%     Participate in treatment teams, and maintain required visits with clients. Other duties as assigned.
