Placement Coordinator
This is a responsible administrative and clinical position calling for the organization and tracking of post-civil commitment placements and other consumers needing inpatient placement within the four-county catchment area.
DESCRIPTION OF DUTIES:
Responsible for developing a working knowledge of all community mental health center operated/contracted residential services in his/her service area relative to admission criteria.
Facilitate placements of consumers needing a crisis residential or inpatient bed.
Gather information, assessments, and hospital records and forward to programs for review.
Coordinate movement and transportation of consumers including obtaining appropriate orders, arrangement of emergency transportation, or if needed, providing transportation.
Maintain an active waitlist of crisis residential beds and ensure that placements are driven by priority.
Serve as liaison between hospitals, community agencies, jails, and other referral sources for consumers needing evaluations and placement within our acute care continuum.
Provide data to the UR Coordinator for admissions and placements and other information is required.
Assist with maintaining DMH Gateway data base for all petitions, post commitment placements and transfers, and discharges.
Participate in clinical staffing to review pending admissions for programs.
Interact with the public, including family members and other collaterals who are seeking placement for an individual.
Assist with the diversion of commitments whenever possible.
Document in the electronic health record when indicated.
Conduct crisis interventions as needed to include co-response to law enforcement officers and crisis calls.
This is a position that requires after hours on-call if evening or weekend placements are needed.
Assist in monitoring the DMH database for all certified residential programs in the Mental Illness Vacancy Reporting system.
Assist in facilitating cross-catchment placements to best meet the individualized needs of the consumers. These placements may be discharges from a state facility or transfers from one residential program to another.
Help facilitate movement out of state facilities by assisting with difficult placements.
Other duties as required.
REQUIREMENTS:
Knowledge and training in mental illness program operations, structure, and development.
Ability to apply programmatic knowledge to the operations of programs for the seriously mentally ill.
Ability to effectively communicate in writing and in person.
Demonstrable communication and interpersonal relationship skills.
Proven writing skills.
Ability to fluently use Microsoft Office Suite.
Reliable transportation.
Willingness to take after hours calls as needed.
QUALIFICATIONS:
Bachelor's degree from an accredited college or university in psychology, social work, or related field plus one year of related experience.
Must hold and maintain a valid driver's license and a driving record that is acceptable to Carastar's insurance carrier.
Must maintain at least liability coverage on personal vehicles.
Must meet requirements for Approved Medicaid Provider status.
