We are seeking a Director of Case Management to join our amazing team at our beautiful and luxurious community of The Bristal Assisted Living at [insert location]. This highly polished professional will serve as the primary liaison between residents, their families and community team members. In this role, you will be the first line of support to residents in navigating the admission and discharge process. This includes providing orientation, social/emotional support, arranging for needed services and maintaining case notes as per company policy and regulatory rule.
Ensuring a seamless, accurate and high-quality admission experience for residents and their families, the Director of Case Management will identify and report changes in resident status and collaborate with property leadership and team members in addressing such changes.
Additional responsibilities include providing specialized in-service training to team members and conducting specialized support groups and/or social education sessions for residents and their families. The Director of Case Management is also expected to maintain a proactive role in cultivating new resident prospects as well as assisting the Director of Community Relations with tours, preadmission paperwork as well as the admission process.
Additional responsibilities include:
- Helping to oversee the Wellness Department, working closely with the Director of Nursing and Director of Reflections (our Alzheimer's/Dementia Neighborhood)
- Assessing prospective resident's physical, mental and emotional needs, as well as identifying current status
- Developing and maintaining accurate client records, logging resident interactions and/or impressions, and maintaining requisite resident case notes
- Coordinating ISP process, incident reporting and the Falls Management Program, as well as coordinating external services, such as rehab, homecare, psychological services
Full-time schedule requires working one weekend day per week (Tues-Sat or Sun-Thurs).
Qualifications:
- Current background in NYS RN, LCSW, LMSW or Social Work
- Experience working in an assisted living, skilled nursing facility or long-term care environment
- Successful background in nursing, case work, social work or health advocacy role
- Minimum of four (4) years' experience with an adult/geriatric population; working in an admissions, assessment, family liaison or discharge planning capacity preferred
- Ability to understand the needs of seniors, including those presenting with dementia
- Impeccable communication skills, both verbal and written
- Sharp attention to detail, exceptional follow-through and time management skills
- Extremely organized, ability to excel in a fast-paced, ever-changing, multitasking environment
- Exceptional interpersonal and relationship-building skills a must, possessing the ability to clearly and calmly relate to and communicate concisely with a diverse group of people
- Demonstrate confident leadership skills
