B. QUALIFICATIONS:

 

A master's degree in a Human Services related field and two years of experience, one of which should be providing clinical services to children/youth with SED and their families. The ability to provide performance-based supervision and support the professional development of others. Experience working with children/youth who have complex behavioral health needs. Willingness and ability to complete HFW Facilitator training. An understanding of and experience with different child-serving systems, including education, mental health, child welfare, juvenile justice, health and others. An extensive knowledge of community resources. Knowledge of agency and federal policies, procedures, and guidelines.  Knowledge of diagnostic impressions or criteria.  Knowledge of how client advocacy system operates.  Knowledge of crisis intervention techniques.  Ability to work and communicate effectively with diverse populations.  Knowledge of individual and group therapy techniques.  Knowledge of various therapeutic models and intervention as directly related to area of assignment.  Knowledge of activities and needs of specialized program.  Ability to develop treatment/care plans and measurable goals.  Ability to work with all parties involved to assist with successful treatment planning.  Ability to make appropriate referrals.  Knowledge of family dynamics.  Knowledge of family support groups.  Ability to facilitate family support groups.  Knowledge of confidentiality policies. Ability to identify potential obstacles through interview and written documentation and assist consumers in overcoming obstacles.  Ability to convey key concepts of behavior change to staff through effective teaching methods.  Ability to identify training needs and ensure necessary job-related instruction is provided.

G.  Program Description

 

Intensive Customized Care Coordination (IC3) is a provider based High Fidelity Wraparound intervention comprised of a team selected by the family/caregiver in which the family and team identify the goals and the appropriate strategies to reach the goals identified by the family. High fidelity wraparound (HFW) is an ecologically based process building on the collective actions of a team to mobilize resources and talents from a variety of sources to support families in their communities. In the wraparound process, a team of people are brought together around all the components of a family's life incorporating their history, culture, relationships, and other relevant information to address their challenges and formulate possible solutions.

 

H. Assigned Duties

 

Under the administrative and clinical supervision of the IC3 Program Director, the Wraparound Supervisor is responsible for providing clinical and administrative supervision, training to assigned staff members and providing care coordination services to clients and families. As the Wraparound Supervisor, you will facilitate staff acquisition and implementation of the skills required in the delivery of DBHDD defined High Fidelity Wraparound services as well as guide staff in the utilization of a team-based, strength-based care approach to service planning. The Wraparound Supervisor provides performance-based, individualized supervision for each staff member at least one hour weekly and group supervision on a monthly basis to provide direction to achieve and sustain high fidelity wraparound services. Supervision includes service plan reviews to determine the progress for each youth and family served. The wraparound Supervisor is responsible for modeling and problem-solving with High Fidelity Wraparound teams to develop strategies to provide culturally competent services, supports and resources for youth and their families

 

Duties Include:

 

Acts as client advocate in order to coordinate required services or to resolve crisis.  Monitors, evaluates and records client progress according to measurable goals described in treatment and care plan.  Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement, or education.  Organizes support groups or counsel family members to assist them in understanding, dealing with, and supporting the client.  Identify environmental impediments to client or patient progress through interviews and review of patient records.  Provides training to direct care staff in implementing behavior health needs of consumers presenting for services.  Provides case management service related to identified treatment needs.  Serves as job expert or organization resource in assigned areas.  Serves as project or team leader.  Performs job responsibilities with minimal supervision. 

 

NOTE:  This position will answer directly to the IC3 Program Director
