High school diploma or GED and one year experience in a social services related position Or Completion of two years of college with at least 15 credit hours in a social services related field.

  1. Performance improvement:  ASPIRE's goal is to continually improve the delivery of service by improvement of individual outcomes and satisfaction.  All employees have a role in performance improvement and are expected to interact collaboratively with co-worker, individuals, and other contacts to prove consistent, high quality, individual-focused services.

 

  1. Age Related and Special needs of population served:

Assist in building recovery-oriented system of care that supports self-directed pathways to recovery by building on the strengths and resilience of individuals, families and communities. Promote long-term recovery from substance use disorders by providing experienced peer support and advocating for self-directed care. Incumbents at this level may require additional training or experience to gain full proficiency in some or all the job responsibilities. 

 

70%     Deliver peer services in MAT program and maintain 70% productivity.  This person will          be responsible for providing peer support (group and individual) to individuals experiencing challenges with opioid use or use or misuse of other substances. Provide support to families, friends and allies who call, uphold the principles and values of recovery in all aspects of services provided, and provide connections to local recovery communities as appropriate and linkages to treatment and recovery support services when requested. Participates in case reviews. Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor.  Provides transportation for clients/residents to appointments and Activities.  Arranges for needed medical care and provides needed services to assure that medical care is obtained. Assists in presentation of educational group lectures related to appropriate topics.    

 

15%     Complete collaborative documentation of service delivery in individual's electronic medical

records incompliance with all ASO and Provider Manual Documentation Guidelines.

 

10%     deliver 100% of authorized services.  Monitors service authorizations to assure services

provided are authorized consults with licensed staff to schedule individual's appointment for reassessment 

 

4%       Attend agency required training, meeting as scheduled by supervisor.

 

1%       other duties as assigned, not to exceed 90 days, as assigned by supervisor. Will keep up with

essential learning to perform assigned work.
