SOURCE CASE MANAGER


 

 

JOB SUMMARY:

The SOURCE Case Manager is responsible for the overall enhanced primary care and case management of those members who have been assigned to their caseload.

Non- Exempt Position.

 

DUTIES AND RESPONSIBILITIES

  • Collects, reviews, and files monthly reports as needed and by the monthly deadline.
  • Maintains and monitors members' files and records by periodically reviewing and following up on reviews.
  • Obtains medical information for all members.
  • Assists members with acquisitions of medications.
  • Monitors the overall health status and wellbeing of the members to ensure that medical and social issues are addressed promptly.
  • Assists the members with doctors' appointments and with following physicians' recommendations.
  • Attends community/agency meetings regarding SOURCE members as needed.
  • Assists members with the maintenance of Supplemental Security Income (SSI) and Medicaid benefits.
  • Collaborates with the SOURCE nurses on specific medical/health issues of concern.
  • Reports all issues of concern to the SOURCE Director.
  • Collaborates with participating physicians and medical facilities to ensure wrap-around case management of members' health needs.
  • Utilizes Therap to input data on members by the end of the service month.
  • Provides and coordinates related services to members as follows:
  •             Transportation
  •             Scheduling medical appointments
  •             Counseling
  •             Information and referral
  •             Serve as On-Call and/or Back-Up
  •             Meals
  •             Emergency Response Systems
  •             Personal Care Home Reside
  •             Other duties as assigned.

QUALIFICATIONS:

Required:  Bachelor's degree in Sociology, Criminal Justice, Psychology, Social Work, Special Education or related field or have experience relevant to the position.  Must have experience working in and with community agencies, with families, and preferable with individuals who have disabilities.
