Licensed Practical Nurse - Adult Foster Care

Job Summary:

Assists supervisor with operations of the Adult Foster Care Program. Ensures AFC program meets regulatory requirements. Contributes to the accomplishment of the Tribe's holistic health and wellness objectives for patients and families.  

Essential Functions:

  • Assists in the management of Adult Foster Care (AFC) resident services; adhering to regulatory principles and the oversight of policies and procedures for the care of the elder and disabled residents.
  • Coordinates AFC activities in accordance with policy and procedure.
  • Maintains and provides all data as requested and/or required.
  • Accurately records patient information in EHR.
  • Contact with other departments/programs to coordinate patient care. Coordinates patient care with other departments.
  • Conducts assessments of potential residents and develops service plans, schedules and updates service information as indicated by resident need.
  • Engages resident and family participation in service/care planning by explaining approaches and involving them in teaching programs. Conduct (or ensure occurrence of) on-going care conferences/assessments as per policies and procedures with family members/resident representatives.
  • Monitors residents' physical and mental functioning psychological well-being and nutritional status to ensure care needs are met in accordance with resident service plans. Notifies manager, physician and family and takes appropriate action when significant changes in condition occur.
  • Participates in written plan of care for each resident and reviews resident's medical nursing treatments to ensure they are in accordance with the resident's care plan and wishes.
  • Performs or coordinates direct resident care as necessary or appropriate.
  • Assists with monthly resident meetings regarding the operation and governance of the AFC facilities.
  • Completes the monthly AFC staffing schedule adhering to policies and procedures and submitting to supervisor for approval by the 15th of the month prior.
  • Provides staff oversight to ensure safe practices are maintained per policy and procedure and reports back to supervisor when concerns arise.
  • Assists with new caregiver onboarding and ensures all tasks are signed off prior to the completion on training onboarding; as well as completes annual skills checklists with caregivers.
  • Performs a variety of other duties as assigned by supervisor.

Education, Experience, and Licenses:

  • Requires a current Oregon LPN license.
  • A minimum of three (3) years' experience as a Licensed Practical Nurse with a minimum of one (1) year experience in a clinical setting preferred.
  • Current CPR/BLS required.

Driving Requirements:

  • This position may require driving.

Qualifications, Registrations, and Certifications:

  • Must be detail oriented with ability to handle high patient volumes.
  • Must have excellent customer service skills.
  • Must have knowledge of Licensed Practical Nursing scope of practice.
  • Ability to operate and maintain medical equipment within the clinic.
  • Must have knowledge and experience in use of computerized electronic health records and software applications.
  • Working knowledge of sterile technique and special procedures applicable to care delivered.
  • Requires working knowledge of sanitation, personal hygiene, and basic health and safety precautions applicable to health clinic setting.
  • Adherences to the organization's policies and procedures covering the privacy, access, and patient health information in compliance with Federal and state laws.
  • Ability to establish and maintain effective working relationships with patients and clinic staff.
  • Native American applicants preferred; knowledge of Native American culture required.

Environmental Factors and Conditions/Physical Requirements:

  • Continually requires the ability to express or receive detailed information or important instructions through oral communication.
  • Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks.
  • Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
  • Continually requires the use of vision, specifically close vision, color identification and depth perception.
  • Occasionally requires lifting of up to 40 pounds.
  • Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.
  • This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.
  • Supports a safe work environment by performing all duties in accordance with Tribal safety policies and procedures, and Emergency Management Plan. 
  • Frequently exposed to bodily fluids and potentially toxic substances.
  • Local travel is occasionally required. Travel outside of the state is infrequently required.
