Referral Coordinator-Bilingual

JOB SUMMARY:

Swope Health emphasizes a team-based approach to health care delivery.  This is accomplished by delivering proactive, planned and coordinated evidence-based care.  We are seeking a bilingual Referral Coordinator.  Successful candidate will accomplish this by their primary function of processing and tracking of referrals to outside ancillary and consult services.

PRIMARY FUNCTIONS:

  1. Ensure the provider has completed all of the necessary information needed to process the referral.Including any forms the specialist may require to be sent in with the referral.

  2. Enter all of the necessary information to complete the referral.

    1. Authorization Code (if the referral requires prior authorization from the insurance company)

    2. Provider or Medical Group the patient is being referred to

    3. Attach the information pertaining to the reason patient is being referred: Medical Summary, Progress Notes, Labs, X-rays and other patient documentation

    4. Insurance 

  3. Coordinate with the Provider/Specialty where the patient is being referred to.

  4. Contact the patient to inform them of the referral

  5. Ensure accurate and timely documentation in the electronic medical record

  6. Track referrals through referral process until results are obtained and entered into the patient medical record

  7. Performa other duties as assigned

QUALIFICATIONS:

  1. High school graduate.

  2. Experience working in a medical office1 year or more

  3. Basic Cardiovascular Life Support (BCLS), certified.

  4. Excellent communication skills, customer service oriented, telephone etiquette and exhibit the ability to work with people (staff, visitors and clients).

  5. Referral Management experience preferred

  6. Fluent in Spanish and English required
