SUMMARY:
This evolving position is located in the Operations Division within the Oklahoma
City-County Health Department (OCCHD). The employee works collaboratively with a
team of Community Health Workers, social service agencies, schools, and agency
programs to provide clients with opportunities to stabilize and improve their
health. As a Community Health Worker (CHW), social and health related needs

supervision of the Community Health Engagement (CHE) Supervisor. The employee
works collaboratively with a team of health care professionals, community health
workers (CHW), social service agencies, schools, and agency programs to provide
clients with opportunities to stabilize and improve their health.
Job Summary:
Contributes to the Tribe's mission of building and enhancing a Tribal Community
rich in healthy families and capable people with strong cultural values by
providing transport services to elderly Tribal members, which affords the
opportunity for them to gather with peers for meals and social activities.
Attends medical appointments supporting adherence to care and treatment, and
empowering individuals to be agents in improving their health and extending
